Office Policies and Patient Information

General Patient Policies

Plan to arrive at least 15 minutes before your appointment so that we may process your paperwork and answer any questions you have. You are responsible for bringing your most recent insurance card(s) and a list of all your current prescription and over-the-counter medications, including dosages.

All patients are required to sign in at the reception desk upon arrival. Your co-pay will be due when you check out.

Eating and drinking are not allowed in the waiting area or in the exam rooms. Cell phones and pagers need to be turned off or put on silent mode while in the exam rooms. Smoking is not permitted inside the building; you may smoke outside in designated areas only.

Abusive language or threatening or violent behavior will not be tolerated in the office. This will be grounds to have you removed from the premises and discharged from the practice.

Appointments, Cancellations and No-Shows

An appointment is required for all services provided. New patients may schedule an appointment by calling our office. Returning patients may request appointments through our Patient Portal.

We will contact you forty-eight hours before your appointment to confirm the date and time. If you need to cancel or reschedule an appointment, we request that you notify us at least 24 hours in advance. This will allow us to offer the time slot to other patients who need to be seen.

After Hours

If you have a life threatening emergency at any time, please call 911 or go to your nearest emergency room.

If you have an urgent condition after hours and must speak with the physician, please call 336-893-8423. You will be prompted on how to leave a message, which will then be forwarded to Dr. Wortman.

Prescription Refills

If you need your prescription medication refilled, please call your pharmacy at least 24 hours before you wish to pick up the medication. The pharmacy will then contact our office for authorization.

If you have not been seen in the office for 12 months or more, you will need to schedule a follow up appointment and be seen by the doctor before further prescriptions will be authorized.

Refills will not be handled after office hours.
